First Grade Report card and Assessment FREEBIE
My first grade report card and assessment kit is finally here!
This has been my most requested resource ever since I created my kindergarten version.
I used to dread assessing my students, documenting all the data and filling out report cards. It was such an overwhelming process, so I finally took the time to create this kit.
It has made my life, and thousands of other teachers’ lives so much easier during quarterly assessment time.

I also have PRESCHOOL, PRE-K & KINDERGARTEN assessment kits and report cards.
I needed one, easy to use assessment binder that correlated with report card forms for each quarter.
I also wanted the report cards to be easy for parents to read and understand so they can quickly and easily understand what skills they student has mastered, and what they still need to work on.
I love that I can easily keep them informed about what we are learning about each quarter in math and language arts. This way, there are never any surprises at conference time.
The report card forms coordinate perfectly with the assessment mats, so I can quickly and easily record their answers directly onto their report cards as I assess them on phonics, reading, math and writing skills.
I spend less than half the time I used to on assessing students and filling out their report cards. This kit has been such a game changer.
I broke the standards down by quarter, and they all follow common core and almost all state standards.
I do not document every single standard, rather the just the main ones we focus on at each point in the year.
I love how easy and streamlined the entire process is, and parents absolutely love that they can quickly and easily see where their child is at each quarter. They can also look ahead to the big goals for each quarter coming up.

Each quarter I assess five main reading and math standards.
I just circle "MASTERED" or "NOT MASTERED" under each goal so parents can quickly see what their child mastered for the quarter.
The other page(s) of the report card go into more detail on each goal, and the assessment kits match perfectly to the report cards.
So if students did NOT MASTER "I can read and spell consonant blends" on their quarter two report card, this second page shows parents exactly which blends they did not know.
It is straight forward for you, students and parents to follow.
As students work their way through the assessment binder, I simply record their answers directly onto their report cards. This makes documenting and keeping track of student data so easy.
The assessment mats and report cards coordinate perfectly making this a seamless process.
A PEAK AT HOW I ASSESS
These kits are pre-made and are not editable.
All standards are common core aligned. The kit aligns with almost all state standards as well, so check out what I assess each quarter so you can see if it aligns with what you teach.
There are assessment mats for each quarter. Simply print, place in page protectors or laminate and you're ready to assess students.
Both the assessment kits and report cards come in color and BW for your convenience.
I use these to assess my students one on one, and the coordinating report cards make this go very quickly.
Because I don't have a full time aide or help, I do a couple activities on the report card whole group with privacy folders. Students can write their names and numbers (depending on the quarter) as a class. This saves a lot of time so when I pull students one on one, I can assess the other standards quickly.
This gives you awesome data and a snapshot of exactly what students know at each quarter in the year.
All the prep work is done for you!
The only thing you need to provide is flash cards of the sight words you're working on and counters/manipulatives for some of the math standards (1:1 correspondence, counting how many, adding/subtracting, etc.) and your reading fluency passages for each quarter.
WHAT IS EDITABLE?
The place for student's name, date and what sight words you're working on each quarter are editable for you to type. Or if you prefer, print the regular copy and you can handwrite these parts in.
The 5 main language arts goals and 5 math goals each quarter are NOT editable.
The pictures and graphics are NOT editable.
This assessment kit tests very specific skills each quarter that fit with most schools/districts.
What if you're working on other standards/goals that are not covered?
I included an editable page for you to type up to SIX additional standards/goals you're working on for each quarter.
This is perfect to add science, social studies, or simply other math and reading goals I did not include for each quarter.
A PEAK AT WHAT IS INCLUDED FOR EACH QUARTER
Each report card then comes with a second page (and sometimes third page) that goes into more detail for each standard and correlates with the assessment kit mats.
SOCIAL AND BEHAVIOR GOALS
This is of course an optional form for you to include with report cards. It comes in BW and color.
The goals are not editable, but the place to type student names, the date and teacher comments can all be done using Adobe 11 Reader.
I feel that self reflection is so important for kiddos to do each quarter! This is a super simple and easy to follow format for children. They are not editable and come in BW. I have my students do these each quarter - they simply color/circle the thumbs up or thumbs down.
STUDENT DATA
Lastly, I included a running record form for you as the teacher to keep track of the standards students have mastered (and which quarter they mastered them in.)
This will remind you which standards you may need to go back and reassess the following quarter if a student did not pass the previous quarter. Instead of wading through a bunch of paperwork, I like to be able to quickly see what standards I need to assess for each student every quarter.
First grade report card testing made easy!
I tried my best to make this report card and assessment kit compatible with any curriculum.
It covers the BIG goals we focus on in first grade, and of course if there are additional standards you wish to include simply use the editable Additional Goals We are Working On form.
For editing, you must use Adobe 11 Reader (free to download at adobe.com) and basic editing directions are included for your convenience.
Please note that the pre-made report cards DO NOT cover every single First Grade Common Core Standard.
SUPPLIES THAT ARE NOT INCLUDED
This kits come with everything you need to assess the standards except the following:
You’ll need to provide your own sight word flash cards. Every teacher/district has their own required sight words, so you can assess students on the exact words you’re working on. Need flash cards? These cards make it easy to target first grade words.
Fluency passages are not included. You’ll need one per quarter to test how many words per minute your student(s) can read. If you do not have any, have your child read from a book at their level for one minute and then count how many words they read correctly.
Math manipulatives: you’ll need coins for quarter 1, a ruler for quarter two, base ten blocks, and optional ten frames and number lines for adding/subtracting within 20.
